This book aims to provide a comprehensive treatment of the author's quantum measurement theory. The reconciliation of the microscopic and the macroscopic - which is the underlying problem of quantum measurement theory - is effected by modifying the foundations of statistical mechanics, not quantum mechanics. A critical examination of those foundations is undertaken and it is shown how, by relaxing assumptions that have never been proved experimentally, it is possible to retain pure quantum evolution in all dynamical systems. (There is no wave function collapse, no nonlinear evolution, no ensembles). The result is a deterministic quantum evolution acting on the wave function of the universe. The theory has experimental consequences, and specific tests are proposed.
